Submit form and payment for child registration

This registration includes:

• Burugar Clinic Training Shirt
• One-off insurance coverage for the year
• All necessary documentation and waivers
• Emergency contact registration

Shirt collected on arrival, Burugar shirt to be worn at all sessions. 

By Registering your child to Burugar Clinic you are agreeing to our program waiver & code of conduct.

Burugar Clinic is committed to creating a safe and positive environment for all participants. However, by signing this form, you acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in sports and physical activity involves risks, including the possibility of injury. I accept full responsibility for my child’s participation and any resulting injury, loss, or damage.

2. Code of Conduct

I understand my child must:

• Follow all rules, safety guidelines, and instructions from staff.
• Treat coaches, volunteers, and other participants with respect.
• Act in a manner that supports a safe and enjoyable environment.

Failure to do so may result in removal from the program without refund.

3. Medical Treatment

In the event of an injury or emergency, I give permission for staff/volunteers to administer first aid and/or seek medical care for my child. I accept full responsibility for any related costs.
